ANSWER: Consent forms do not release health care providers when malpractice is involved. The healthcare provider who performed a negligently is still libel. It needs to be established that the health care provider did not follow the applicable standard of care and the patient's injury or disease was caused or aggravated substantially because of their actions. If the health care provider went beyond your consent, they may also be liable for battery.
